Preparing For Home

Planning for discharge is an important part of a patient’s stay in hospital. Patients are advised to discuss the following important issues with their doctor, nurse and family:

  • Transport arrangements to return home
  • Capacity to care for personal needs
  • Capacity to care for others at home
  • Capacity to care for the home environment
  • Readiness to resume usual work or social activities.

Please discuss any concerns with our nursing staff, so that the necessary arrangements can be made and we can assist with future care.

Staff can assist in organising equipment for use at home, such as walkers, crutches and toilet chairs.

Discharge Process

Discharge is on or before 10:00am as the rooms are scheduled for re-use after 11:00am.
On the morning of discharge, please visit reception to ensure that all necessary paperwork is completed prior to discharge.

Patients who are unable to be picked up by 10:00am are welcome to wait in the lounge areas until they are able to picked-up. A taxi service can be booked through Reception. (NB: this will be at your own expense). To prevent delays it is important that the patient informs staff of their discharge date to allow administration staff enough time to finalise their account ready for discharge.

Prior to leaving, patients are reminded of the following:

  • Pack all medications, repeat prescriptions, and x-ray films
  • Check the room thoroughly for all personal belongings
  • Receive a Discharge Summary, which outlines follow-up care
  • Advise Reception of departure
  • Finalise your account with the administrative team.
